I also was hoping to preserve a folder structure - for example, under Movies, I have a folder called "TV show 1" and under it, a bunch of episodes from that TV show. I thought I would dump my photos in the Photos subdirectory, and my videos in the Movies subdirectory, and the clients would see them respectively under each subfolder. I've made two subfolders under Multimedia: one for photos and one for movies. Did this and dms-1 saw it just fine and played music with no problems. I choose to have it install as a service but my dms-1 wouldnt see it until i read on the their site that you should run the musicserver.exe file from the same dir as your music.
